Denise Ann Onofrio of Lawrence, Kansas was involved in a crash on Friday, October 30th 2020 at 9:00 am in Douglas County, Kansas. Onofrio is a 59-year-old woman.

In the crash, Onofrio was driving a 2016 Kenworth Semi Truck. The crash happened here: US Highway 40 at mile marker 384.9 Westbound or 0.4 miles West of US Highway 59 on the South edge of Lawrence. The police described the crash like this:

Vehicle one was traveling Westbound on U40 at mile marker 384.9. A deer crossed the roadway headed North. Vehicle one struck the deer, the deer flipped up into the windshield. Vehicle one came to a stop on the right shoulder.

Injuries

The police said that Onofrio had possible injury (complaint of pain). Onofrio was taken to Lawrence Memorial Hospital.

Vehicle Damage

The police said the damage to the 2016 Kenworth Semi Truck with Kansas license plate number 149922 was not stated. It was insured by Ace American Insurance Company. It was removed from the crash scene by making own arrangements. The police said: Trucking company made their own arrangements for a tow.

How To Get the Highway Patrol Report

The Kansas State Highway Patrol Trooper with badge number K396 wrote up the report. The highway patrol assigned case number to this crash. The KSHP charges $5.00 for each copy of the report plus additional fees of $2.00 for each witness statement. But you can call 877-925-1969 to request a free copy of the report. In order to get the report through the KSHP website, you must create a Kansas.gov account and give them your credit card. You must agree to be subject to Kansas Statute 45-230: Unlawful Use of Names Derived from Public Records. This statute imposes on those subject to it "a civil penalty in an action brought by the attorney general or county or district attorney" of up to "$500 for each violation."
