Matthew David Zaleski of Southfield, Michigan was involved in a crash on Tuesday, December 18th 2018 at 4:28 pm in Ellis County, Kansas. Zaleski is a 43-year-old man.

Vehicle was westbound on I70 at milepost 165 when the driver lost control and the vehicle entered into the median. Vehicle turned broadside and rolled two times in the median and then came back onto the roadway in the east bound lanes and rolled three times crossing the east bound lanes.Vehicle came to rest on its wheels in the south ditch and the driver was ejected from the vehicle.

Vehicle Damage
The police said the damage to the 2005 Chrysler PT Cruiser with Michigan license plate number MW8QB96 was not stated. It was insured by Pioneer State Mutual. It was removed from the crash scene by Reins and taken to lot.
How To Get the Highway Patrol Report
The Kansas State Highway Patrol Trooper with badge number K281 wrote up the report. The highway patrol assigned case number 2018-018429 to this crash. The KSHP charges $5.00 for each copy of the report plus additional fees of $2.00 for each witness statement. But you can call 877-925-1969 to request a free copy of the report. In order to get the report through the KSHP website, you must create a Kansas.gov account and give them your credit card. You must agree to be subject to Kansas Statute 45-230: Unlawful Use of Names Derived from Public Records. This statute imposes on those subject to it "a civil penalty in an action brought by the attorney general or county or district attorney" of up to "$500 for each violation."
